Cagrilintide is a long-acting acylated analogue of amylin — the pancreatic hormone co-secreted with insulin at mealtimes — with a half-life of approximately 7–8 days enabling once-weekly dosing. It activates central amylin receptors in the area postrema and hindbrain to promote satiety, slow gastric emptying, and reduce food intake independent of the GLP-1 pathway. In Phase 2 trials at 4.5 mg weekly, cagrilintide produced approximately 10.8% body weight reduction over 26 weeks. When combined with semaglutide 2.4 mg in Phase 3 REDEFINE trials, the combination produced approximately 20% weight loss at 68 weeks — exceeding either agent alone.
