Overview
Retatrutide is an investigational triple-receptor agonist peptide (GLP-1, GIP, glucagon). Phase 2 trials demonstrated up to 24% body weight loss at 48 weeks. This protocol uses 1.0 mL reconstitution for ~5.0 mg/mL. At 5.0 mg/mL, 1 unit = 0.01 mL ≈ 50 mcg on a U-100 syringe. For doses >5 mg, reconstitute multiple vials.

Reconstitution Instructions
Draw 1.0 mL bacteriostatic water with a sterile syringe.
Inject slowly down the vial wall; avoid foaming.
Gently swirl/roll until dissolved (do not shake).
Label and refrigerate at 2–8 °C (35.6–46.4 °F), protected from light. Use within 4 weeks of reconstitution.

Dosing Protocol
Schedule
Inject once weekly subcutaneously. For doses >5 mg, reconstitute multiple vials (each with 1.0 mL) and draw the required volume. Volumes >1.0 mL may be split into 2 injections at different sites.
Titration
Start at 2 mg weekly for 4 weeks. Increase by 2–4 mg every 4 weeks as tolerated. Gradual titration is critical to minimize GI side effects.
Cycle Length
Minimum 24 weeks; clinical trials extended to 48 weeks. Same day each week.
Storage & Handling

- ✓Reconstituted: Refrigerate at 2–8 °C; use within 4 weeks.
- ✓Allow vials to reach room temperature before opening to reduce condensation.
